We certify qualified Hawaii residents for Medical Use of Marijuana (MUM). 

State certified patients
can legally grow up to seven plants at a time and possess reasonable amounts in their own home.

  • Cancer
  • Seizures
  • HIV
  • Glaucoma, and others


The Dept. of Health Medical Marijuana Registry has a NEW registration process. It’s entirely electronic! Simply put, you will need an email address to file an application.

Tips for Applications:

  • Your name MUST MATCH YOUR ID EXACTLY. This is very important.
  • The ID Number is your Driver License, State ID or Passport number. Make sure that matches EXACTLY.
    • All Hawaii driver license start with an H followed by ALL NUMBERS. Not the letter “O”
    • Make sure the ID number matches the ID if it is from out of state.
  • The Expiration Date field on the applicaton is your ID's expiration date.
    • Any TEMPORARY ID being used MUST have the temporary exportation date.
  • SUFFIX is for Jr, Sr, or III (only if on ID) NOT for Mrs, Mr, etc...
  • Your address MUST contain Rd, St, Hwy, Ln, Blvd, etc.
  • Check for the correct island under residential address. **MAUI or HAWAII**
  • Check the GROW SITE BOX if you are or ever plan on having a grow site at your home or other address.
  • If you have a CAREGIVER:
    • Ensure all of their information is current and correct.
    • Print caregiver certificate and grow-site certificate to be signed in the appropriate places by applicant and caregiver.
    • Either attach signed forms to the application or bring them to your appointment.
    • Either attach a copy of the caregiver’s government issued ID or bring the copy with you to your appointment.

RECERT APPLICATIONS - Up to 60 days before going expired. *COMPLETE BEFORE CALLING*

  • Sign into https://medmj.ehawaii.gov with your Email address and Password
    • *Be forewarned that they will prompt you to change your password. This can be as simple as adding an ! or ?, or feel free to create a whole new password*
  • Once you are signed in to your account click 'Application Status' and look for the RENEW button.          
    • It should auto-fill the information from your application from the previous year.
    • Update any information that needs to be updated such as your address, ID expiration, grow-site, etc…
  • Proceed to the end of the application.
  • Pay the yearly $38.50 application fee to the Department of Health using a credit/debit card.
  • Call or e-mail us with your APPLICATION NUMBER to schedule your appointment:

NEW APPLICATIONS:
**DO NOT COMPLETE UNLESS YOU HAVE CALLED TO BE PRESCREENED FIRST**

  • Go to https://medmj.ehawaii.gov and click the ‘CREATE FREE ACCOUNT’ link.
  • Make yourself an ‘egov’ account (Keep your password handy and do not lose it).
  • You will be logged into the https://medmj.ehawaii.gov site with your e-mail and the password you created.
  • Fill out the online application starting with your physician’s name: CHARLES WEBB.
  • Continue to fill out the application with your information, caregiver information (If applicable. Must have signed papers and ID for caregiver), and your grow site or opt to not have a grow site.
  • Upload your ID to the application. If you are unable to upload your ID please text or e-mail us a picture of the ID to 808-430-4763 or mumclinic@aol.com  
  • Pay the yearly $38.50 application fee to the Department of Health using a credit/debit card.
  • Write down your APPLICATION NUMBER and call or e-mail us to schedule your appointment.

**To make any changes to your 329 card (MMJ Card) or to request a replacement due to a lost or stolen card, go to:

**Print the ‘Change Form Packet’ for the Deptartment of Health, mail it to the Deptartment of Health with all updated and correct information, a photocopy of your government issued ID, and a money order (if needed).
