State certified patients can legally grow up to ten plants at a time and possess reasonable amounts in their own home.

New patients MUST provide documentation at their first appointment.

The Department of Health has an electronic registry and requires an online application that is done annually.

Please READ or PRINT instructions THEN click the link to the DOH

  1. Sign into https://medmj.ehawaii.gov with your email address and password (the ones you used from the prior year).
    1. Be forewarned that they will prompt you to change your password. This can be as simple as changing the special character or a number. OR feel free to create a whole new password.
    2. If you do not have your password from the year before you will need to click RETRIEVE PASSWORD link to get a temporary one sent to you email.
  2. Once you are signed in to your account click 'Application Status' and look for the RENEW button.
    1. Press the renew button. It should auto-fill your information from the previous year.
    2. Update any info that needs to be updated, such as: your address, ID expiration date, grow-site, etc.
    3. We can help you attach your ID at the office.
    4. If you have a caregiver make sure to update any of their info along with getting the print out pages (caregiver certificate and grow site certificate, 3 total) to have them sign. MAKE SURE TO READ THROUGH AND CHECK OFF THE APPROPRIATE BOXES. Yes, you have to do this every year.
      1. You can grab them, to print, on the Upload Documents page by pressing the box that says ‘Get it here’.
      2. Either attach them yourself or bring the signed pages to your appointment along with both valid IDs. (If IDs are still current then we will have them on file from the previous year.)
        1. If you have a NEW caregiver then we will need their ID.
  3. Proceed to the end of the application, sign the Terms and Agreements then pay the annual $38.50 application fee, to the Department of Health, using a credit/debit card.
  4. Call or e-mail us with your APPLICATION NUMBER to schedule your appointment:

NEW APPLICATIONS

  1. Go to https://medmj.ehawaii.gov and click the ‘CREATE FREE ACCOUNT’ link.
    1. Make yourself an ‘egov’ account using your email address and make up your own password. (Write your password down and do not lose it.)
  2. Once your account is set up you will need to sign into https://medmj.ehawaii.gov and fill out the application.
  3. CHARLES WEBB is the first and last name of the physician to be filled in at the begging.
  4. Continue to fill out the application with YOUR information. Double check details as you go. Errors on the application can delay getting your card.
  5. Caregiver information (if applicable) must be current and fully filled out on the appropriate page. Also, you must have the signed papers and IDs for both of you. (Papers can be grabbed from the ‘Get it here’ button on the upload documents page, three pages total.) READ EVERYTHING CAREFULLY.
    1. If you cannot attach signed papers you will need to bring them to your appointment.
    2. We need both IDs prior to the appointment. Either attach them or text/email them to us.
  6. Check off grow site box or opt to not have a grow site.
  7. Upload a picture of your ID.
    1. If you cannot attach the ID then you must send it to our email mumclinic@aol.com or text them to us at 808-430-4763
  8. Scroll to bottom of the last page and select the type of card for the payment then read through and e-sign the ‘Terms and Agreements’
  9. Pay the annual $38.50 application fee to the Department of Health using a credit/debit card.
  10. Write down your APPLICATION NUMBER and call 808-430-4763 or email mumclinic@aol.com us to schedule your appointment.

Tips for Applications:


*To make any changes to your 329 card or to request a replacement due to a lost
or stolen card you will need to fill out and mail in a ‘Change Form Packet'*

Health with all the correct information, a photocopy of your government issued ID, and a money order $16.50(if needed).
